Dermal filler can be a beautiful and effective way to restore volume, support facial structure and soften the signs of ageing. When it is placed well and in the right amounts, it should look natural and feel like a seamless part of your face. However, filler does not always behave perfectly over time. Products can shift or start to look unnatural, when placed without respecting the anatomy. At Illuminate Skin Clinic, we often see patients who would benefit more from dissolving old filler than adding more.
Understanding when filler needs to be dissolved is just as important as knowing when to have it injected. Here are five common signs that it may be time to consider starting again with a cleaner, more balanced foundation.
You’re seeing a blue or grey tint
The Tyndall Effect is a telltale sign that dermal filler may need to be dissolved. It happens when hyaluronic acid filler is injected too close to the surface of the skin. Light hits the filler and scatters, creating a tint beneath the skin. On fair complexions, this usually appears blue; on deeper skin tones, it can look grey or shadowy. Either way, it creates an unnatural, bruised appearance that can be hard to conceal.
It’s most common under the eyes or in areas with thin skin, where incorrect placement is more noticeable. While not medically dangerous, the Tyndall Effect is a visible red flag that filler has been placed too superficially or has migrated. Over time, it can lead to long-term cosmetic concerns such as persistent discolouration, puffiness, and uneven texture. The filler may attract water, causing swelling or a puffy, distorted look, especially in delicate areas like the tear troughs. Left untreated, it can continue to affect facial harmony and draw attention to the very areas you hoped to enhance.
Your features look distorted or less like you
Filler should enhance your natural features, not change them. When too much product has been added over time or when filler has migrated from where it was originally placed, the face can start to look unfamiliar. Cheeks can look too full, lips can lose their shape and the lower face can appear bulky.
If you feel that you no longer recognise yourself in the mirror or that your face looks overdone even when you have not had treatment for a long time, dissolving old filler can restore balance and allow us to rebuild in a more natural way if needed.
You can feel lumps or unevenness under the skin
Smoothness is a hallmark of good filler work. If you can feel lumps, ridges, or firm areas beneath the skin, it suggests that filler has clumped together, shifted or been placed in a way that is no longer sitting well within the tissue.
This can happen months or even years after treatment as the face moves and ages. In these cases, dissolving the filler is often the best way to create a smooth, even surface again before considering any new treatment.
Your filler no longer looks as good as it once did
Filler does not always simply fade away. Over time, it can break down unevenly, leaving behind irregular volume or subtle swelling. Many people come to us saying their filler used to look great, but now looks tired, heavy or undefined.
If you feel your results have declined rather than just faded, dissolving and resetting the area is often the safest and most effective option. This allows us to see your true anatomy again and decide what, if anything, is needed to enhance it.
You’ve had multiple top-ups over the years
Repeatedly adding filler to the same areas when it isn’t necessary is one of the most common reasons faces start to look unnatural. Filler can accumulate, migrate and alter the way tissues move and drain. This is why someone who has only ever had small amounts can still end up looking overfilled after years of maintenance.
If you have had regular filler for a long time, dissolving can be a proactive way to protect your long-term facial balance. It gives us a clean slate and allows us to rebuild with a lighter, more precise approach.
Is dissolving filler safe?
Filler is dissolved using an enzyme called hyaluronidase, which breaks down hyaluronic acid safely and effectively. When used by an experienced medical practitioner, it is a well-established and controlled procedure. Some swelling and tenderness is normal afterwards, but most patients are surprised by how quickly their face settles and how refreshed they look once excess filler has gone.
At Illuminate Skin Clinic, we take a careful, conservative approach to dissolving. The goal is not to remove everything unless necessary, but to restore harmony and allow your natural features to shine again. It may require several sessions to gain the desired results, as treating slowly and safely is most important.
In many cases, dissolving filler is the most intelligent and empowering step you can take for your face. Whether you are dealing with puffiness, distortion, lumps or simply results that no longer feel right, removing old filler can give you back control over how you look.
